Section He-C 6450.18 - Case Records on Juveniles(a) Each juvenile's case record shall have weekly entries on observations made on the juvenile's response to individual goals and objectives identified in the treatment plan.(b) Each juvenile's record shall contain copies of monthly progress reports sent to DCYF and parents that address the information identified in He-C 6350.03(y) .(c) The record shall contain treatment plans filed in accordance with He-C 6450.17.(d) Separate reports in the juvenile's file shall document all accidents, personal injuries, and behavioral incidents.(e) Educational requirements shall be documented and shall include progress with stated educational objectives as set forth in the individualized education plan.(f) A record of the juvenile's academic achievements shall be maintained for a receiving school district to interpret educational attainment for the purpose of grade assignment.(g) Summaries of parent or legal guardian contacts shall be entered in the case record.(h) The case record shall document: (1) Any psychological, medical, or dental services provided;(2) Any time spent away from the physical plant by the juvenile;(3) Updates or corrections of any information received by the experiential/wilderness facility that pertains to the care of the juvenile; and(4) Discharge information.(i) All information relating to a juvenile in placement shall be confidential, pursuant to RSA 170-E:42. This information shall not be released except to the legal representative of the juvenile, the parent, or legal guardian of the juvenile, the JPPO, or the authorized agency having custody of the juvenile.(j) Any other release of information shall require a court order from a court of competent jurisdiction.(k) All visits and contacts shall be subject to any limitations agreed upon at the time of admission, and as later modified through the treatment plan.N.H.
