Section He-C 6450.17 - Discharges(a) Prior to discharge, the staff shall develop a transition plan in conjunction with the JPPO, that specifies the services to be provided to the juvenile and family to support family reunification.(b) Discharge of juveniles shall be in accordance with the following: (1) Discharge of the juvenile shall be planned during the admission process, with the child, parent, legal guardian, or the JPPO or the juvenile for aftercare planning;(2) At least one month prior to the juvenile's planned release, a discharge notice shall be given to the parent or legal guardian, the court which placed the juvenile, the JPPO, and the individual or agency having legal custody of the juvenile for coordination of the juvenile's continuing treatment needs upon leaving the experiential/wilderness facility;(3) Discharge shall be made only to the person having legal custody of the juvenile, the JPPO, or to an authorized representative of the agency having legal custody of the juvenile; and(4) Discharge of a juvenile shall be documented in the juvenile's file and shall comply with He-C 6350.45(c) -(e) .(c) The written discharge summary shall be completed within 15 days after the juvenile's discharge, and a copy shall be sent to the JPPO.N.H.
