Section He-C 6450.16 - Treatment Planning Process
(a) Experiential/wilderness facilities shall conduct a psycho-social assessment within 30 days of admitting a child and develop a treatment plan on the basis of the assessment.
(b) The treatment plan shall include, but not be limited to:
(1) The findings of the assessment;
(2) The goals and objectives to be achieved by the child and family;
(3) The time frames for goals and objectives and the method to be used for evaluating the child's progress;
(4) The long term plans for the child;
(5) The chil d's daily activities, including education and recreation, to be provided by staff;
(6) The services that will be provided directly or arranged for, and any measures for ensuring their integration with the child's activities;
(7) The goals and preliminary plans for discharge;
(8) An identification of the persons responsible for coordinating the implementation of the treatment plan; and
(9) The date and signatures of:
a. The resident director or designee;
b. A representative of DCYF; and
c. When applicable for medicaid funding, the prescribing practitioner, child, and the child's parents and/or guardian.
(c) The treatment plan shall be implemented by the treatment team/program staff and shall be reflected in the daily routine.
(d) Revisions of treatment plans shall be explained in writing.
(e) The treatment team shall consist of the following:
(1) The child;
(2) The child's parents and/or guardian;
(3) A representative of DCYF;
(4) Sending school district personnel as determined by the school districts;
(5) The prescribing practitioner;
(6) A treatment staff member from the experiential/wilderness facility; and
(7) Other persons significant in the child's life, including but not limited to:
a. Teachers;
b. Counselors;
c. Friends; and
d. Relatives.
(f) When any of the treatment team members listed in (e) do not participate, the staff shall document its efforts to involve them and forward a copy of the treatment plan to them.
(g) A treatment plan meeting shall be held a minimum of once every 4 months for evaluating progress made towards the established goals and objectives.
(h) A case conference shall be held every 6 months to update the treatment plan and review the requirements as stated in He-C 6350.08(c) - (g) .
(i) If the experiential/wilderness facility is providing services for referrals made by DCYF, the treatment planning process shall also comply with He-C 6350.08.
