Section He-C 6450.14 - Admissions(a) Prior to admission, the resident director shall secure documentation of legal custody, as well as any court-ordered plan which requires specific services for the juvenile.(b) The experiential/wilderness facility shall admit the juvenile only upon written agreement signed by the person or referring representatives having legal authority to place the juvenile.(c) Each juvenile shall have a medical assessment conducted by a physician that includes a written statement indicating the juvenile's ability to participate at the experiential/wilderness facility and the absence of communicable diseases at the time of admission.(d) The resident director shall obtain written educational information from the juvenile's home school to determine the level of instruction needed.(e) If an individualized education plan has been developed by the juvenile's school district, it shall be implemented based on the requirements in RSA 186-C.(f) Upon admission of a juvenile, the experiential/wilderness facility's files shall include the following in writing: (1) Who has legal custody, the address and telephone number of the court, and the name of the juvenile's JPPO;(2) The name of the person or entity having financial responsibility for the support, and medical and dental care of the juvenile, as well as their addresses, and telephone numbers;(3) The court order for the juvenile;(4) Any individualized education plan developed for the juvenile by the school district;(5) Any court or referring agency limitations placed upon calls, letters, and visits;(6) Provisions, if any, for clothing allowances; and(7) Written consent from the juvenile's parent, legal guardian, or person designated by the court to sign a consent for the juvenile to participate in activities which take place away from the physical plant. (f) The information required for admission of the juvenile shall: (2) Include: a. Background information on the juvenile including:1. Name, address, race, sex, religion, birth date, and place of birth of the juvenile;2. Name, address, telephone number, and marital status of the parent and/or legal guardian, siblings, grandparents, and other significant individuals to the juvenile;3. Date of admission, source of referral, and the name of the JPPO; andb. A copy of the juvenile's birth certificate.N.H. Admin. Code § He-C 6450.14
