Section He-C 6450.03 - Application Process
(a) Inquiries concerning licensing of experiential/wilderness facilities and requests for copies of requirements and/or assistance shall be made to:

(b) All applicants shall complete an application form provided by the department and submit it to DCYF within 90 days of receipt of the application form.
(c) The application shall be granted or denied by DCYF within 60 days of the date the completed application is received.
(d) DCYF shall extend this period for an additional 30 days when unusual circumstances, beyond the control of the applicant, prevent completion within the time allowed.
(e) The applicant shall provide the following on the application:
(1) The type of application, new, renewal or revision;
(2) The current date;
(3) The name and address of the experiential/wilderness facility;
(4) The name and business telephone number of the director;
(5) The number of juveniles to be cared for;
(6) The age range of the juveniles;
(7) Organizational charts and employee job descriptions;
(8) The name and address of 3 non-related persons for references on the director, including the most recent employer, if applicable;
(9) The names of all employees in order to review criminal records and the DCYF child abuse registry;
(10) The names, addresses, and telephone numbers of the members of the board of directors and advisory board;
(11) Indication of approvals for the physical plant by the local fire department or state fire marshal's office and the local public health officer; and
(12) Signature of the resident director.
(f) Those persons named as references on the application shall be contacted for the purpose of verifying the qualifications and experience of the resident director.
(g) In accordance with RSA 170-E:29, II, DCYF shall, upon receipt of a new application for a license, review the criminal conviction records of the state police for all persons who are employed by the experiential/wilderness facility in any capacity.
(h) In accordance with RSA 170-E:29, II, DCYF shall, upon receipt of a new application for a license, review the child abuse records containing child abuse reports for all persons who are hired by the experiential/wilderness facility.
(i) If any of the following situations occur for any applicant, employee, or substitute staff member, DCYF shall immediately review the status of the license by conducting an investigation to determine if further licensing measures are needed to protect the juveniles:
(1) A child has been removed by the court from his or her custody because of abuse or neglect;
(2) A petition is pending in court for abuse and/or neglect charges;
(3) Conviction of a crime against a child, or awaiting a trial on such charges;
(4) Receipt of protective services for self and/or family members through DCYF or an agency providing similar services in another state; or
(5) Identification as a perpetrator of abuse or neglect in the state central registry of child abuse/neglect reports, or in a similar registry in another state.
(j) The applicant shall determine and comply with zoning requirements, when appropriate, and documentation of compliance shall be available at the physical plant.
(k) The experiential/wilderness facility shall have a detailed written procedure for fire and emergency, approved by the state fire marshal.
(l) Copies of fire and emergency procedures shall be available at the physical plant.
(m) Documentation of fire drills conducted shall be contained in the experiential/wilderness facility's records.
