Each household participating in a qualifying housing assistance program that is interested in enrolling in Career MAP shall complete the Department's application for the Career MAP lottery.
The application shall include questions that require the household to attest to the Program eligibility criteria listed in § 13001, and shall also request the applicant to provide the following:
Except as provided in § 13004.4, only the head of household described in § 13001.1(e) shall be eligible to apply for the Career MAP lottery on the household's behalf.
An authorized representative may apply on behalf of the household, if the head of household provides the Department with a written and signed statement including the name and address of the person authorized to act on the household's behalf.
The Department shall assist any head of household or authorized representative with the application process upon request.
As part of the Career MAP lottery application, each head of household or the household's authorized representative, shall sign a release form authorizing the Department to obtain or verify information necessary for processing the application, confirming eligibility, and evaluating the Program.
The Department shall review applications and enter households meeting the eligibility criteria for the Career MAP lottery into one or more assignment lotteries to determine which eligible households the Department may offer an available funded Program slot to. This lottery process may be combined with lotteries for other programs at the discretion of the Department.
The Department shall notify households determined ineligible for the lottery of the determination.
After the lottery is completed, the Department shall conditionally offer available Program slots to households selected by the lottery.
The Department shall notify selected households in writing of their conditional offer to enroll in the Program.
The offer of a Program slot is conditioned on the household responding to the Department's written notice of the conditional offer and successfully completing the Program eligibility verification and enrollment process described in § 13005.
Each household selected in the lottery shall have fifteen (15) calendar days from the date on the written notice of conditional offer to respond to the notice. Each household's response to the Department shall convey whether the household:
A household shall lose its conditional Program slot, and the next household selected in the lottery will be offered the slot, if the first household:
If a household loses its conditional Program slot, the Department shall notify the household in writing that its conditional offer has been withdrawn.
If new Program slots become available, the Department shall reopen the lottery process and may either accept new applications for the lottery or make conditional enrollment offers to households that were previously determined eligible for the lottery.
