Rule 29-13005 - ELIGBILITY VERIFICATION AND PROGRAM ENROLLMENT
13005.1

The Department shall verify the following eligibility information for each household offered conditional Program enrollment:

(a) District residency;
(b) Household composition, including age of each household member; and
(c) Participation in qualifying housing-assistance program.
13005.2

The Department may consider current or recent participation in a government-funded emergency or temporary housing or rental assistance program administered by the District as verification of each of the eligibility criteria reflected in Section 13005.1 to the extent that participation in that program requires verifying those criteria.

13005.3

The Department shall first attempt to verify the information in Section 13005.1 using third-party data sources to which it has legal access, which may include using remote and in-person interviews to verify Program eligibility.

13005.4

If the Department cannot independently verify an eligibility criterion, it shall request documentation of that eligibility criterion from the household and provide the household at least ten (10) days from the date of the request to provide the documentation.

13005.5

The household shall be responsible for timely providing all documentation the Department requests from it to verify eligibility.

13005.6

The Department shall notify the household once all requested documentation needed to verify eligibility has been received.

13005.7

If a household has not obtained and provided to the Department the requested information needed to verify eligibility for the Program within thirty (30) calendar days of the date of the notice of conditional offer or the due date provided in a notice requesting documentation, whichever is later, without an extension for good cause granted by the Department, the household shall lose its conditional Program slot.

13005.8

The Department or a service provider shall contact each household that has completed eligibility verification to schedule an intake meeting.

13005.9

At the intake meeting, the head of household shall complete assessments, interviews, questionnaires, and other activities to establish career and household goals and needs, which the Department shall use to customize support services.

13005.10

At the conclusion of the intake meeting, the head of household shall sign an initial mobility action plan.

13005.11

Once a head of household signs the initial mobility action plan and has had eligibility confirmed by the Department, the household has completed all steps necessary for enrollment in the Program.

13005.12

Once a household completes the enrollment process, the Department shall facilitate the household's disenrollment from any other District or federal government-funded rental assistance program to ensure the household's compliance with the eligibility requirements set forth in § 13003.1(a). Once this disenrollment is complete, the household shall be an enrolled participant in the Program, shall receive written notice to confirm enrollment, and shall begin receiving support services through the Program.

13005.13

At the discretion of the Department, a household may, if it demonstrates good cause, receive an extension of time to respond to the notice of conditional offer or provide requested documents to verify eligibility.
