Rule 10 - Form of Pleadings(a) Caption; title. Every pleading shall contain a caption setting forth the name of the court and probate district, the title of the proceeding, the docket number and the title of the pleading. The title of the proceeding shall include the name of the decedent or proposed ward and an appropriate phrase describing the type of proceeding. The petition shall be dated. (b) Forms. A pleading must be submitted on an official form contained in these rules, if applicable, unless the court directs otherwise.
