Rule 3.12 - Voluntary Surrender of Certification, Registration, or License(a) A regulated person may voluntarily surrender the person's certification, registration, or license. The surrender must be submitted to the Director on the form approved by the Commission, and the form must be completed fully. The Director may request additional information.(b) Except under circumstances addressed under (d), a voluntary surrender is effective after the Director receives it, verifies that the form is complete, and changes the person's status on the Commission's website.(c) The person must provide written notice of the voluntary surrender as required by the Director.(d) If a regulated person is a respondent in a disciplinary matter under investigation or before the Commission at the time of surrender, the circumstances under which the certification, registration, or license is circumstances under which the certification, registration, or license is surrendered will be noted in the person's records with the Commission and will be taken into account if the person applies in the future. A voluntary surrender for purposes of settling a disciplinary matter is not effective until the Commission approves the settlement as a final resolution of the disciplinary matter.Tex. R. Judi. Cert.
