If a proposed facility is to be used for supplying water with which to fill a reservoir, an application should be submitted to cover this use. The same application form as that used for an original water right application (Form S.W.
1) must be used if an original facility is involved. If an existing ditch or other facility is involved, an enlargement application (Form S.W.
2) must be used. The application should be filled out in the same manner as the original water right or enlargement form except that: - a. ITEM 3 -- ''The use to which the water is to be applied is . . . ." -- State the use to which the water is to be applied, such as "Supply (or additional supply) for Model Reservoir, Permit No. 100 Res." If for additional supply, the estimated quantity of water to be furnished from this source should be stated.
- b. The map accompanying the application for reservoir supply must snow a cross-section of the supply ditch or other facility and appropriate hydraulic properties. If the reservoir is to be supplied by s lateral from the primary diversion facility, then a crosa-section of such lateral and hydraulic properties must be shown on the map to establish the rate at which the off-channel reservoir may be filled.
- c. ITEM 10 -- "The land to be irrigated . . . ." - The tabulation of lands to be irrigated should not include any land description unless there should be some new lands which are irrigated directly from the supply ditch. Merely include a statement at the top of the apace devoted to this tabulation, such as "Supply for Model Reservoir, Permit No. 100 Res." See sample application, page 43.
