No person may begin injection or construction, installation, modification, or operation of a facility which may result in well injection into, above, or through an underground source of drinking water without a permit to do so from the board. Application for a permit shall be submitted on forms provided by the secretary. The permit shall not be issued until the application has been completed by the owner or operator of the site. Plans and specifications for the injection facilities shall be submitted to the secretary in accordance with chapter 74:53:04. Data used in completing the application shall be retained for at least three years after the date of submission.
