Application forms for a well pump installer's license shall be furnished by the chief engineer upon request or submitted electronically. All written applications submitted to the chief engineer shall be typewritten or printed in permanent ink and shall be legible. A well pump installer's license representative shall be designated in the application. The license representative shall be responsible for all well pump installation and repair work contracted for or authorized on behalf of the well pump installer. The license representative's qualifications and experience shall be the basis for documenting in the application the ability of the well pump installer to properly install well pumps.
