Written personnel policies and records for all staff must be maintained by the center and include provisions for equal employment opportunities. Each center must maintain a personnel file or record for each staff including contracted staff, intern, or volunteer. The file or record must include at a minimum the following:
(1) Individual personnel records must include an application filed for employment or resume and transcripts or diploma and continuing education;(2) A position description signed by the staff with a statement of duties and responsibilities and the minimum qualifications and competencies necessary to fulfill these duties;(3) The completion of appropriate pre-hire screening will be evident for staff that provide direct services to vulnerable populations;(4) Documentation of the staffs orientation in accordance with § 67:62:06:05; and(5) Copies of the staffs current credentials related to job duties.S.D.
