Section 67:46:01:11 - Application for assistance

Except for those individuals covered under subdivisions 67:46:01:02(2), (3), (8), (12), (22), or (24), an individual desiring medical assistance under this article or someone acting on the individual's behalf, must submit a completed and signed application for assistance to the department.
