Section 67:16:46:04 - Limit on hours of service

A diabetes self-management training is limited to the following:

(1) Ten hours of initial education and

(2) Two hours a year of follow-up education.

The department must authorize services that exceed these limits before the services are provided. Authorization is based on documentation supplied to the department that justifies the need for the additional services.
