An individual is eligible to participate in a diabetes self-management training if the individual is not institutionalized, the program is provided by a provider eligible under the provisions of § 67:16:46:02, the individual has not previously received diabetes self-management training, and the individual meets one of the following conditions:
(1) The individual is a newly diagnosed with diabetes or gestational diabetes;(2) The individual demonstrates poor glycemic control as evidence by glycated hemoglobin level more than 2 percent above the upper limit of normal for the assay used;(3) The individual's treatment regimen has been changed by a physician or other licensed practitioner;(4) The individual has documented episodes of acute, severe hypoglycemia or hyperglycemia occurring in the past year that required third-party assistance; or(5) The individual is at high risk because of the presence of extremity, renal, or cardiac complications or diabetic retinopathy.S.D.
