A Certificate of Contract Completion shall be prepared by the design consultant using the form prescribed by the Department. The contractor shall complete the Certificate of Contract Completion and return to the design consultant. The total of the contract shown on the certificate shall include the sum of all change orders. The contractor shall secure and transmit to the design consultant a completed and notarized Consent of Surety with the contractor's Certificate of Final Payment.
