Any application submitted pursuant to the authority of this Part shall be in the form prescribed by Supervisory Procedure CB 105 of this Title, except that the fee accompanying such application shall be in the amount specified in section 1.2 of Supervisory Policy G 1 of this Title in the case of a transaction to which one or more of the parties is a stock-form thrift institution. The application should be sent to the division of the Banking Department that supervises the acquiring or receiving institution at the address of the department set forth in section 1.1 of Supervisory Policy G 1.
