Section Pos 901.03 - Application(a) Each applicant shall complete an application, form PEC 900, to participate under RSA 188-D:18, I, in the workforce incentive program.(b) The applicant shall supply the following information on form PEC 900: (1) His or her first name and last name;(2) Applicant's current address;(3) Applicant's current employer;(5) Beginning and ending date of service for which loan repayment is sought;(6) Name and title of certifying official;(7) Signature of certifying official; and(8) Educational loan information as follows: a.Date, amount and interest rate of each educational loan;b.Balance of each educational loan;c.Name of servicer of each educational loan;d.Address of servicer of each educational loan.(c) The application shall be signed and dated by the applicant.(d) The applicant shall attach a copy of the most recent loan payment invoice(s).N.H.
