Section Pos 801.04 - Conditions of Grants to Institutions(a) Grants to institutions under the leveraged incentive grant program shall be made only to eligible institutions which shall have executed a grant agreement with the commission.(b) The grantee shall make institutional matching funds of its own available for expenditures under the leveraged incentive grant program on behalf of full-time undergraduate students who are New Hampshire residents and who have completed their freshman year.(c) The institutional matching funds shall:(1) Be from sources of funding raised on or after July 1 of the current fiscal year;(2) Not have been pre-committed to the institution prior to that date; and,(3) Not be reallocated institutional funds.(d) The commission shall make funds available to participating institutions based on the distribution formula.(e) The distribution formula shall be based on the relationship of the enrollment of the institution's undergraduate students who have completed their freshmen year to the total enrollment as follows:(f) For purposes of (e) above: (1) An undergraduate student who has completed his or her freshmen year means those students, other than freshmen, who were reported on the prior year = s fall enrollment survey form of the Integrated Postsecondary Education Data System (IPEDS) as seeking a degree or certificate at an accredited college within the state; and(2) Total enrollment means the total number of students, as defined above, enrolled in all accredited institutions in the state.(g) After the close of the fiscal year in which a grantee receives funds under the leveraged incentive grant program the grantee shall complete and forward to the commission's office a matching fund report form, PEC form 801 containing the following information: (1) The name of institution;(2) The amount of funds accepted;(3) The amount of matching funds raised;(4) The name of authorized official; and(5) The title of authorized official.(h) The matching fund report form shall be signed and dated by the certifying official.N.H.
