Section Pari 806.34 - Waiver of Use of Application Form for Occupational Licensees

The commission shall grant an applicant a waiver from the use of the form described in Pari 806.05 provided that the applicant submits to the commission any occupational license application form produced by the Association of Racing Commissioners International Inc., together with:

(a) A written submission of any additional information required by the form noted at Pari 806.05;

(b) The applicable form noted at Pari 806.07 - Pari 806.27; and

(c) The items required by 806.03 (a) (3) - (4).
