Section He-E 502.31 - Transportation(a) Transportation services shall be covered for the purpose of accessing the following types of services: (1) Title III services, except home delivered meals;(2) Medical appointments;(3) Shopping for groceries and other basic needs; and(4) Services provided by community facilities and agencies that increase participation in programs, or otherwise promote independent living.(b) Contract agencies providing transportation services shall comply with provisions included in the contract with regard to routes and reimbursements.(c) Transportation services shall be provided in vehicles that are: (1) Registered pursuant to Saf-C 500;(2) Inspected pursuant to Saf-C 3200, and are in good working order; and(3) Insured for personal liability.(d) Transportation services shall be provided by individuals who: (1) Have a current and valid driver's license; and(2) Are employees of a Title III service provider or other transportation provider, or volunteers under the supervision of a Title III service provider.(e) Contract agencies shall document transportation services provided to the individual on an operational schedule or on a service provision log that includes: (1) The date(s) of service;(2) The starting and ending locations;(3) The name(s) of the individual(s);(4) The reason the transportation services are required;(5) The name(s) of the driver; and(6) Comment on any follow-up service provided.(f) Transportation services for individuals shall not be reimbursed when duplicative of any other program or services, or when included as a core service under the rate paid to the service provider.N.H.
