Section He-E 502.30 - Prevention Services(a) Prevention services shall be covered for individuals who meet the eligibility requirements for Title III services as described in He-E 502.05.(b) Prevention services shall include, but not be limited to: (1) Evidence-based health screenings that can detect the presence of, or an individual's risk for, heart disease, diabetes, cancer, asthma, strokes, vision loss, hearing loss, or other chronic diseases or conditions;(2) Evidence-based group educational programs or individual counseling on topics such as nutrition, exercise, mobility, medication management, pain management, home safety, the emotional aspects of chronic disease or conditions, or other related topics;(3) Individual assessments and the development of individual action plans to help prevent injuries or prevent or manage chronic diseases or conditions; and(4) Referrals to other service providers as necessary, including health care providers who can follow up on further prevention or treatment of chronic diseases or conditions.(c) For each individual receiving prevention services that include individual counseling, assessments, or action plans: (1) An assessment shall be completed and documented;(2) Goals shall be identified and an individual treatment plan developed; and(3) Progress notes shall be made by the counselor following each contact or session.(d) Contract agencies providing prevention services shall keep a service provision log for health screenings, referrals, group educational services, or walk-in service activities that includes: (1) The date of the activity;(2) The type of activity or service provided;(3) The name(s) of the attendee(s) or service recipient(s); and(4) Comment on any follow-up service provided.N.H.
