Section He-C 801.04 - Option to Opt Out(a) Notwithstanding the requirements of He-C 801.03, above, licensed health care providers shall have the option to opt out of the survey process.(b) Licensed health care providers choosing to opt out of the survey shall complete and submit the "New Hampshire Health Professions Survey Opt-Out Form" (December 2019) located on the Health Professions Data Center at .(c) The form shall be submitted to SORH via one of the following: (1) Emailing as an attachment to the relevant address, as follows:a. For advanced practice registered nurses, NHAPRNSurvey@dhhs.nh.gov;b. For physician assistants, NHPASurvey@dhhs.nh.gov;c. For physicians, NHPhysicianSurvey@dhhs.nh.gov;d. For mental health practitioners, NHMHPractitionerSurvey@dhhs.nh.gove. For drug and alcohol counselors, NHLADCSurvey@dhhs.nh.gov; andf.
