Section He-C 6450.10 - Reinstatement(a) To have a license reinstated after revocation, the applicant shall submit a new application and submit to the entire licensing process as if no license had previously been issued.(b) Pursuant to He-C 6450.07(d) . a new application shall not be accepted from an applicant who has been denied licensure or for whom license renewal has been refused unless the applicant provides documentation showing elimination or correction of the conditions that led to the revocation or refusal to renew.(c) For suspensions, a license shall be reinstated after the expiration of the suspension period.N.H.
