Section He-C 6354.03 - Monitoring of Home-Based Therapeutic Services(a) Quality, performance, and need for the providers of home-based therapeutic services shall be reviewed by DCYF and DJJS annually, pursuant to RSA 170-G:4, VI and RSA 170-G:4, XVIII.(b) Purchased service providers shall be monitored and evaluated by the DCYF service utilization reviewer through a variety of means, including: (1) Monthly queries of data that is stored on NH bridges case management system and the medicaid management information system;(2) Quality assurance review of case record information;(3) Data reporting from the service providers, as described in (c) below;(4) Quality assurance surveys from the families served, as described in (e) and (f) below; and(5) Surveys from and discussion with the CPSW or JSO who authorized the service.(c) Service providers shall complete the home-based therapeutic services data reporting form each month that includes: (1) The reporting month and year;(2) The name of the agency and the agency's contact person;(3) The name of each family served, sorted alphabetically by last name;(4) The NH bridges case number assigned to each family;(5) An indication of compliance with each of the following 5 indicators: a. Any new incidents of abuse or neglect that resulted in a founded determination;b. A child was placed outside the home during service provision;c. During service provision, a child in the home committed a new juvenile offense that was found true at adjudication;d. The child was safely reunified with the family within 3 months of service provision when the goal of the purchased service was family reunification; ande. The family signed the treatment plan, pursuant to He-C 6352.21(t) .(d) Prior to service termination, the service provider shall supply each family with the "Home-Based Therapeutic Services Family Survey".(e) The parent shall complete the home-based therapeutic services family survey shall by providing: (1) The full name of the family and the home telephone number;(2) The name of the therapist and the agency that provided home-based therapeutic services;(3) The dates that purchased services began and ended;(4) Indication of the primary reason that purchased services were provided to the family;(5) An indication of whether: a. The therapist explained: 1. The services that would be provided to the family;2. That services were available 24 hours a day, 7 days a week;3. The cost of the purchased services; and4. Reimbursement for purchased services;b. If crisis services were provided, did the therapist contacted the family within 24 hours of the request;c. The therapist helped resolve the problems in the family;d. Communication has improved in the family;e. Family relationships have improved;f. The services helped to prevent future problems in the family;g. If a child in the family was temporarily placed in foster care, the therapist provided support to the family;h. The person completing the survey knows what agencies in the community can help the family in the future;i. The therapist showed care and respect to the person completing the survey and other family members; andj. The person completing the survey was satisfied with the services that were provided;(6) Any written comments; and(7) The printed name of the family member who completed the survey or the individual who assisted and the date.(f) The parent shall return the home-based therapeutic services family survey to the DCYF service utilization reviewer at state office.(g) The service provider shall assist the parent in recording the survey's answers, if necessary.(h) As measured by the provider during service provision, the outcomes for home-based therapeutic services shall be as required in He-C 6352.21(al) .(i) The provider shall demonstrate the degree of compliance with the outcomes required by He-C 6352.21(a1) (1) through (3) by reporting the data as described in He-C 6354.03(c) .(j) As measured by monitoring by the DCYF service utilization reviewer after termination of service, the outcomes for home-based therapeutic services shall be as required by He-C 6352.21(an) .(k) Agency compliance with the indicator required by He-C 6352.21(an) (1) a. shall be measured by: (1) A query run each month to obtain names of cases with closed home-based therapeutic services authorizations;(2) A review of identified cases that have founded assessments;(3) A review of new assessments;(4) Counting any new incident of previously identified abuse or neglect against agencies in figuring the % of families; and(5) A discussion of case record information with the CPSW.(l) Agency compliance with the indicator required by He-C 6352.21(an) (1) b. shall be measured by: (1) A query run each month to obtain the names of cases with closed service authorizations;(2) A review of identified cases that had a child placed;(3) Obtaining the placement type and reason; and(4) A discussion of case information with the family services CPSW.(m) Agency compliance with the indicator required by He-C 6352.21(an) (1) c. shall be measured by: (1) A query run each month to obtain the names of cases with closed service authorizations;(2) Obtaining offense type, not obtained from the query, from case record information;(3) A review of identified cases in which a juvenile has re-offended; and(4) A discussion of case record information with the JSO.(n) Agency compliance with the indicator required by He-C 6352.21(an) (2) shall be measured by: (1) A query run each month to obtain the names of cases with closed placement and removal data;(2) A review of identified cases that had a child placed outside the home;(3) Identifying placement type and reason from case record information; and(4) A discussion of the case record information with the family services CPSW.(o) Agency compliance with the indicator required by He-C 6352.21(an) (3) shall be measured by: (1) The results of survey from each family who has a closed service authorization;(2) A query run each month for the names of cases closed and the names of the CPSW or JSO; and(3) A review of completed staff surveys.(p) The home-based therapeutic services provider shall meet or exceed each indicator's percentage established in He-C 6352.21(al) and (an) .(q) If any indicator's percentage falls below the required percentage, a service monitoring team, as described in (r) below, shall meet each quarter to:(1) Review areas of non-compliance with He-C 6352;(2) Determine corrective action to be taken by the agency; and(3) Make recommendations for corrective action plans and revocation of certification.(r) The home-based therapeutic services review team shall include: (1) From the bureau of quality improvement, one representative;(2) At least 2 program administrators; and(3) The DCYF service utilization reviewer.(s) A report to agencies shall be provided by the DCYF service utilization reviewer if problems are noted on the staff surveys that include: (1) Negative responses concerning quality and timeliness of service provision; and(2) Written comments about agency performance.(t) An annual report of statistical information shall be prepared by the DCYF service utilization reviewer for the administration and for each home-based therapeutic services agency regarding the results of monitoring.N.H.
