Section He-C 6349.09 - Review of Continued Certification Compliance(a) All providers shall complete a review form, as provided by DCYF, within 30 days of the receipt.(b) Providers that do not submit a review form within 30 days of receipt shall have their certification revoked in accordance with He-C 6349.18 and denied payment.(c) Continuance of certification shall be based on a review and verification of the provider's compliance with service requirements.(d) Review of continued certification compliance shall occur every 2 years from the date of issue for licensed-exempt providers and shall coincide with the licensing date for licensed providers.N.H.
