Section He-C 6344.30 - Denial of Application and Revocation of a Medical School Program Provider Certification

An application shall be denied or certification revoked if:

(a) DCYF or DJJS determines that the state does not have a need for the service.
(b) The applicant or provider, or the individual acting on the applicant's or provider's behalf, submits materially false information to DCYF or DJJS.
(c) There has been a loss of accreditation.
(d) There has been failure to comply with He-C 6344.
