Section He-C 6344.29 - Denial of Application and Revocation of a CMHP Provider Certification

An application shall be denied or certification revoked if:

(a) There has been failure to comply with He-C 6344.

(b) There has been failure to comply with He-M 426.

(c) There has been termination of the contract with the BBH.
