Section Gen 403.03 - Reinstatement of Licenses Lapsed For No More Than 70 DaysThe board shall reinstate a license lapsed for no more than 70 days if the applicant for reinstatement submits to the board:
(a) A completed, dated, and signed reinstatement application form as further described in Ahp 601.02 provided by the genetic counseling governing board;(b) The reinstatement fee set forth in Ahp 301.02(a);(c) Evidence of good professional character and reliability as shown by: (1) The answers to the questions from the reinstatement application form specified in Ahp 601.02 provided by the genetic counseling governing board;(2) Any detailed reports of the relevant circumstances related to answers to those questions on the reinstatement application form specified in Ahp 601.02;(d) The statement required by Gen 403.07(b);(e) Proof in accordance with Gen 406 of having completed any requirement for maintenance of continuing competence left uncompleted at the time the license lapsed;(f) A copy of the applicant's current certificate issued by one of the following: (3) The successor organization.(g) Any documentation required in Gen 303.04 which was not previously provided to the board.N.H.
