Rule 40-1-4.1 - Requesting an Evidentiary HearingA. The following procedures shall govern formal hearings and requests for reconsideration.B. Any person, firm or corporation that has a license of permit suspended or revoked may request an evidentiary hearing within fourteen (14) days from the date of the Commission action.C.
