OVR/OVRB, with input from the State Rehabilitation Council, will maintain for each applicant and eligible individual a record of services that includes, to the extent pertinent, documentation including, but not limited to the individual's application for VR services, the eligibility/ineligibility determination, the individual's Individualized Plan for Employment (IPE), and information related to the closing of the service record.12
