(1) An application for a permit shall be filed with the department in accordance with this Regulation for all elevator work covered under these rules.(2) Each application for a permit shall be accompanied by copies of specifications and accurately-scaled and fully-dimensioned plans that clearly indicate location of the elevator in the building; the location of the machinery room and the equipment to be installed, relocated or altered; all structural supporting members, including foundations; and a specification of all materials to be used and all loads to be supported or conveyed. The plans and specifications shall be sufficiently complete to illustrate all details of construction and design.(3) The applicable fees shall accompany each permit application.(4) A permit may be revoked for any of the following reasons: (a) Where any false statements or misrepresentations were made as to the material facts in the application, plans, or specifications on which the permit was based.(b) Where the permit was issued in error and should not have been issued in accordance with this chapter.(c) Where the work detailed under the permit is not being performed in accordance with the provisions of the application, plans, or specifications or with this chapter or conditions of the permit.(d) Where the elevator contractor to whom the permit was issued fails or refuses to comply with a STOP WORK order.(5) A permit expires if the work authorized by a permit is not commenced within twelve (12) months after the after the date of issuance.(6) A permit is not required for a repair.(7) The Administrator may allow an extension of time at his or her discretion.Miss.
