Section XLIX-1113 - Duplicate LicensesA. Upon receipt of satisfactory evidence that a license or certificate of registration has been lost, mutilated, or destroyed the board may issue a duplicate license or certificate upon such conditions as the board may prescribe, and upon payment of a fee as determined by the board.La.
