Section XV-745 - Surveys After Source Implant and RemovalA. Immediately after implanting sources in a patient or a human research subject, the licensee shall make a survey to locate and account for all sources that have not been implanted.B. Immediately after removing the last temporary implant source from a patient or human research subject, the licensee shall perform a radiation survey of the patient or human research subject with a radiation detection survey instrument to confirm that all sources have been removed.C. Before releasing a patient or a human research subject treated with a remote afterloader unit from licensee control, a licensee shall survey the patient or the human research subject and the remote afterloader unit with a portable radiation detection survey instrument to confirm that the sources have been removed from the patient or human research subject and returned to the safe shielded position.D. A licensee shall maintain a record of patient or human research subject surveys that demonstrates compliance with Subsections A, B, and C of this Section for three years. Each record shall include the date and results of the survey, the survey instrument used, and the name of the individual who made the survey.La. Admin. Code tit. 33, § XV-745
