Section 05.02.01.120 - ADMINISTRATIVE RECORDS01.Documentation Retention. The Provider must document and retain documentation of all information related to the following items: a. Program consultation provided, such as technical assistance on program design and implementation;b. Training provided to staff;c. All alleged instances of child abuse;d. Program audits or reviews, including corrective actions required and taken;e. Reports of sexual abuse disclosures to the applicable state licensing authority or law enforcement;f. Juvenile offender and staff grievances;g. Copies of all completed incident reports; andh. Copies of background checks for all current employees, contractors, volunteers and interns who may have contact with residents.02.Employee Files. Employee personnel files must contain the following:a. Minimum qualifications for the job held;c. Copies of all required licenses or certificates related to the job function;d. Copies of academic credentials, driving record, and background checks, as required by state law;e. Current training records; andf.
