Within six months after the completion of any well or completion of any deepening, redrilling, plugging, altering, or abandonment work, the operator shall file with the department of land and natural resources in Honolulu, Hawaii, the following well reports on forms provided by the department:
(1) Drilling log and core report. The drilling log and core report shall show the lithologic characteristics and depths of formations encountered, the depths and temperatures of ground water-bearing and geothermal resources-bearing strata, and the temperatures, chemical compositions, and other chemical and physical characteristics of fluids encountered from time to time, so far as ascertained. The report shall show the depth, lithologic character and fluid content of cores obtained, so fas as determined.(2) Well history report. The well history report shall describe in detail the chronological order on a daily basis all significant operations carried out and equipment used and shall be submitted upon completion of drilling, testing, completion, recompletion and abandonment of a well.(3) Well summary report. The well summary report shall show data pertinent to the condition of a well at the time of completion of work done. Well locations shown on this form shall be surveyed by a licensed surveyor.(4) Supplementary notice. Reports on any other operations not specifically mentioned herein which affect the previous reported status of a well shall be reported on the supplementary notice form.(5) All reports shall be the property of the state with the right to utilize the same.[Eff.
