Both current and newly enrolled IFS Waiver participants who meet the requirements of § 11100.2 may elect to enroll in the My Life, My Way program.
The Department on Disability Services (DDS) staff provide information regarding self-direction and the My Life, My Way program to all current IFS Waiver participants and to new IFS Waiver participants at the time of IFS Waiver enrollment, as outlined in the approved IFS Waiver.
DDS Service Coordinators provide information on self-direction and the My Life, My Way program to all IFS Waiver participants who are not enrolled as My Life, My Way participants when:
If an IFS Waiver participant expresses interest in the My Life, My Way program, the participant's service coordinator shall be responsible for providing an overview of My Life, My Way program requirements to interested participants and overseeing the participant's completion of all forms and develop an initial ISP to include the My Life, My Way program.
All Service Coordinators shall be required to complete a standardized training course on self-direction and the My Life, My Way program conducted by DDS prior to enrolling participants into the My Life, My Way program, as well as all ongoing training required by DDS.
Upon completion of the documents referenced in § 11105.4, the Service Coordinator shall submit the natural supports/Emergency Back-up Designation form and all required documents to Support Broker.
Enrollment in the My Life, My Way program shall only occur following DDS approval of the participant's PDS budget as described in § 11113 and issuance of a prior authorization for all PDS services and individual-directed goods and services included in the approved budget.
Participants shall be notified at the time of enrollment in the My Life, My Way program that participation in the program is conditional upon compliance with all program rules and the terms of the Participant/Representative-Employer Agreement.
