Rule 25-G607 - ISSUANCE OF LICENSE - NOTICE OF OPENING, DISCONTINUANCE OF OPERATION, AND POSTINGS
607.1

An operator shall provide notice to the Department of its intent to operate the establishment at least thirty (30) calendar days before beginning operations.

607.2

An operator shall provide notice to the Department of its intent to shut down permanently. The operator's license and certificate of occupancy shall be returned to the Department and the owner shall be required to submit a new application for the issuance of a new license prior to reopening.

607.3

An operator shall notify the Department at least thirty (30) days in advance of its intent to close temporarily.

607.4

A current inspection report, all valid licenses, a Certificate of Occupancy, including the "Age Restriction Signs" required in Subsection 201.3, and the "Required Disclosure" required in Subsection 202.3 shall be conspicuously posted in the reception area next to the body art establishment's license.
