The grant application shall require at a minimum a statement of student needs used to determine the program being submitted for funding, the capacity of the staff for the program to deliver the services, a description of how the program will be evaluated for effectiveness including measurable objectives, the hours of operation, and the number of children to be served. If the program is being implemented by a qualified community based organization a description of that organization's capacity and history of delivering such programs shall be included.
