Section 660-1-4-.02 - Obtaining Copies Of Forms

(1) Single copies of forms may be obtained, without cost, by written request or contact with the State Office or main county offices of the Department.

(2) A list of internal forms excluded from the Departmental Forms Index and single copies therefrom may be obtained, without cost, by written request or contact with the State Office.
