Section 660-1-4-.01 - Departmental Forms Index

(1) The Departmental Forms Index is hereby promulgated as a rule of the Department. The index, Appendix B, contains a listing of all forms used by the Department to inform and otherwise impart information to the public, to solicit, to gather, to verify, to certify and to record information required of and from the public by the various programs of the Department. Internal forms used for Departmental management, control, operations and communication are excluded from the index.

(2) Appendix A is provided to facilitate the use of the index. Appendix A includes:

(a) Abbreviations used in the index; and

(b) Organization of the index by Departmental divisions and program/service areas.

(3) The index will be updated on a calendar quarter basis.
