On page 13, by adding a new paragraph 6.2.a.3.G., to read as follows:
6.2.a.3.G Failure to cooperate with OHFLAC or the designated Medicare Beneficiary and Family Centered Care - Quality Improvement Organization is grounds for disciplinary action and further review by the Board. And,
By renumbering the remaining paragraphs.
