Section 522.064 - Notification of Previous Employment and Offenses(a) A person who applies for employment as a commercial motor vehicle driver shall provide the employer, at the time of the application, with the following information for the 10 years preceding the date of application: (1) a list of the names and addresses of the applicant's previous employers for which the applicant drove a commercial motor vehicle;(2) the dates between which the applicant drove for each employer;(3) the reason for leaving the employment of each employer; and(4) each specific criminal offense or serious traffic violation of which the applicant has been convicted and each suspension, revocation, or cancellation of driving privileges that resulted from the conviction.(b) The applicant must certify that the information furnished is true and complete. An employer may require an applicant to provide additional information. Before an application is submitted, the employer shall inform the applicant that the information provided by the applicant under this section may be used, and the applicant's previous employers may be contacted, to investigate the applicant's work history.(c) An employer shall require each applicant to provide the information specified by Subsections (a) and (b).Tex.
