Section 256.054 - Application for Examination To take the examination under Section 256.055, an applicant for a license must:
(1) submit an application on a form prescribed by the board;(2) pay the application fee set by the board;(3) attach to the application:(A) proof of current certification in cardiopulmonary resuscitation approved by the American Heart Association or American Red Cross; or(B) if the applicant is not physically able to comply with the certification requirements of Paragraph (A), a written statement describing the person's physical incapacity executed by a licensed physician or an instructor in cardiopulmonary resuscitation approved by the American Heart Association or American Red Cross; and(4) provide any other information the board requires to determine the applicant's qualifications.Amended by Acts 2001, 77th Leg., ch. 1420, Sec. 14.081(a), eff. 9/1/2001.
