Section 206.214 - License Holder Information(a) Each license holder shall file with the medical board:(1) the license holder's mailing address;(2) the address of the license holder's residence;(3) the mailing address of each of the license holder's offices; and(4) the address for the location of each of the license holder's offices if that address is different from the office's mailing address.(b) A license holder shall:(1) notify the medical board of a change of the license holder's residence or business address; and(2) provide the medical board with the license holder's new address not later than the 30th day after the date the address change occurs.Added by Acts 2001, 77th Leg., ch. 1014, Sec. 1, eff. 9/1/2001.
