Section 2522 - Application and required documentation(a) Form.--An application for licensure as a provider must be in a form prescribed by the department.(b)Fee and documentation.--An application for licensure as a provider must be accompanied by:(1) A licensing fee established by the department.(2) A bond that complies with section 305.
