Section 11701.105-A - ApplicationA program application must be submitted by the applicant municipality on a form prescribed by the department utilizing the electronic single application format and include or demonstrate all of the following:
(1) The name and address of the municipality or, in the case of a multimunicipal application, the municipalities.(2) The name of a contact person.(3) The execution of a supporting resolution authorizing the submission of the application and committing the resources of the municipality or, in the case of a multimunicipal application, municipalities.(4) The single application shall be signed by the authorized officer of the municipality or, in the case of a multimunicipal application, municipalities.(5) Any other information required by the department. 1987, July 10, P.L. 246, No. 47, § 105 -A, added 2014, Oct. 31, P.L. 2983, No. 199, § 7, effective in 60 days [Dec. 30, 2014].
