(a) Substance of forms.--(1) An advance health care directive may be in the form provided under Subchapter D (relating to combined form) or in any other written form that contains the information required under Subchapters B (relating to living wills) and C (relating to health care agents and representatives).(2) A Commonwealth agency that licenses health care providers or regulates health care may not prescribe a mandatory form of an advance health care directive.(b) Combining forms.--A living will and health care power of attorney may be combined into one document.2006, Nov. 29, P.L. 1484, No. 169, §4, effective in 60 days [ 1/29/2007].
