Section 13:1E-99.49 - Request for information by departmenta. Any person claiming an exemption pursuant to subsection a. of section 5 of this act shall maintain on file, and shall transmit, in writing, to the department upon its request, the following information:(1) A statement setting forth the specific basis upon which the exemption is claimed;(2) The full name, business address, telephone number and signature of the person claiming the exemption; and(3) The full name, business address and telephone number of the authorized local representative of the person claiming the exemption.b. The information required pursuant to this section shall be furnished to the department for each specified package or packaging component requested by the department.
